0:07.3 | And today I'm going to cover Cimetidine, which is the brand name of Tagamint. |
0:13.9 | Now, this medication is classified as an H2 receptor antagonist. |
0:20.7 | And if you remember mechanistically, |
0:24.6 | so parietal cells are in the gut, the stomach, |
0:29.6 | and they secrete stomach acid, which ultimately lowers the pH. |
0:34.6 | And histamine actually binds on those receptors and stimulates them to produce that |
0:43.7 | acid. So by being a histamine 2 receptor blocker, we can actually prevent, reduce the amount of |
0:53.8 | stomach acid that is secreted. |
0:57.8 | Okay, so that's how that drug works to reduce stomach acid. |
1:02.6 | So you could imagine what this medication would primarily be used for, |
1:07.7 | any type of situation that you've got an excess of stomach acid and or that acid is causing |
1:15.9 | damage, discomfort, pain, things like that. So GERD, if you've got some sort of peptic ulcer, |
1:23.1 | things of that nature, acid secretion disorders, where the body produces excessive amounts of acid |
1:32.3 | potentially. And a good example, that is Zollinger-Ellison syndrome. Those are good examples of what |
1:40.0 | this medication might be used for. One of my greatest frustrations with cymetidine is that this |
1:49.2 | drug is available over the counter. I rarely, rarely see it used. I can't recall the last time I had |
1:56.0 | seen it prescribed, but I did want to cover it specifically because it is available over the counter. |
2:02.6 | And the reason why it's not used is because it has a ton of drug interactions. |
2:08.8 | But patients who may not be that savvy, they see acid reducer on the counter, over the counter, |
2:16.1 | and they may pick it up and start using it. |
