Hello and welcome to the zero to finals podcast. My name is Tom and in this episode I'm going to

0:09.0

be talking to you about thyroid function tests. And if you want to follow along with written notes on

0:14.0

this topic, you can follow along at zero to finals.com slash tfts or in the endocrinology section of the zero to finals medicine book.

0:24.5

So let's get straight into it.

0:27.3

Thyroid stimulating hormone or TSA.

0:31.1

If you're concerned about possible thyroid disease,

0:34.3

you can use TSA alone as a screening test. When the thyroid stimulating hormone is

0:40.9

abnormal, then you can measure T3 and T4 to find out more information. In hyperthyroidism,

0:48.7

TSA is suppressed by the high thyroid hormones, so you get a low TSA level.

0:55.3

The exception is in a pituitary adenoma that's secreting TSA, in which case the TSA level will be high.

1:02.6

In hypothyroidism, the TSA level is high as it's trying to stimulate more thyroid hormone release.

1:10.8

The exception to this is when the pituitary gland or the hypothalamus is causing the hypothyroid.

1:16.9

And this is called secondary hypothyroidism, in which case the TSA level will be low,

1:22.2

because it's the low level of TSA level that's causing the low level of thyroid hormone.

1:28.5

T3 and T4.

1:31.1

In hypothyroidism, you expect raised T3 and raised T4 and suppression of T-SH.

1:38.9

In hypothyroidism, you expect low T3 and T4 and raised T. Auto-antibodies

1:48.0

Auto-antibodies indicate that there's an autoimmune condition

1:53.0

that's causing the pathology in the thyroid gland.

1:56.0

Antithyroid peroxidase or anti-tpo antibodies are antibodies against the thyroid gland itself.

2:04.3

And these are the most relevant thyroid auto-antibody in autoimmune thyroid disease.

2:10.1
