Summary

In this episode, I discuss loperamide (Imodium) pharmacology, adverse effects, and drug interactions.

Loperamide has opioid-type activity in the gut but has extremely low oral bioavailability. This allows it to be used for diarrhea but at lower doses won’t cause systemic opioid-like effects.

Loperamide abuse has been reported. Excessive dosages can increase the risk of cardiac arrest and other cardiovascular concerns.

Medication causes of diarrhea should be ruled out prior to starting a medication like loperamide. I discuss numerous medications that can cause diarrhea in this podcast.

All right, let's get into the drug of the day today, and that is lopuramide.

Brand name of this medication is Emodium.

You may also see it listed as Emodium AD.

So the AD obviously stands for antidiaryl.

So as you can imagine, this drug is used to manage symptoms of diarrhea.

Most common situations that I see it used in or idiopathic diarrhea where we

basically can't identify a cause, but the patient kind of leans towards loose stools or has lots

of loose stools. Certainly IBS is a common syndrome that can have diarrhea symptoms associated with it,

so I've certainly seen it used there periodically on an as-needed basis.

Oncology, medications, adverse effects from those,

classic one being erinotiken causing diarrhea diarrhea other kind of rare situations short bowel syndrome so

there's lots of potential causes of diarrhea and symptomatically lopuramide can potentially

help with that a little bit more more, I would say, controversial.

I have seen some clinicians use it, and I have seen others steer away from it,

but it's use in viral or bacterial infections.

Again, kind of controversial, whether it should or shouldn't be used.
