Hi, this is Tom, and in this episode, I'm going to be going through hypoxic ischemic encephalopathy,

0:10.7

and you can find notes at zero to finals.com slash h-I-E or in the zero-to-finals pediatrics book.

So let's jump straight in. Hipoxic ischemic encephalopathy, or H-I-E, occurs in neonates as a result of

0:33.5

hypoxia during birth. Hypoxia is a lack of oxygen.

0:39.8

Ischemia refers to a restriction in blood flow to the tissues.

0:45.6

Encephalopathy refers to the malfunctioning of the brain.

0:51.1

Some hypoxia is normal during birth, however prolonged or severe hypoxia leads to ischemic brain injury.

1:01.9

H.I.e. may result in permanent neurological impairments, including cerebral palsy, developmental delays, cognitive deficits, or in severe cases, death.

1:15.5

Let's talk about the causes.

1:18.8

Anything that leads to asphyxia or deprivation of oxygen in the brain can cause hypoxic

1:26.0

ocemic ancphalopathy.

1:30.4

For example, maternal shock,

1:38.0

intrapartum hemorrhage, a prolapsed cord causing compression of the umbilical cord during birth, and nucal cord, which is where the cord is wrapped around the baby's neck.

1:45.6

Next let's talk about diagnosis.

1:49.0

Factors that suggest possible hypoxic ischemic encyclopathy include hypoxia during the perinatal or intrapartan period,

2:00.2

acidosis with a pH below 7 or a base deficit above 12 to 16

2:06.4

millimoles per litre on the umbilical artery blood gas, poor APGAR scores, for example, less than

2:14.7

five at 10 minutes of age, and features of hypoxic ischemic encephalopathy,

2:21.3

which will go through shortly.

2:24.4

An MRI scan and electroencephalography, or EEG, may be used to help establish the diagnosis.
