In today's podcast, we're going to be addressing herpes and the USMLEs. So we're going to call this the high-yield herpes podcast for the USMLA exams.

0:16.3

This stuff is incredibly high-yield to know for step one, step two, and step three. And it's also incredibly high-yield to know for level one, step two, and step three. And it's also

0:22.0

incredibly high yield to know for level one, level two, and level three. So we're going to run

0:26.2

through the high yield things you need to know, make a bunch of very nice integrations, and try

0:30.4

to prepare you really well for your exams. And one thing I would like to say is that our friends

0:36.3

at the MBMEs, they really love.

0:38.3

So if you see me say the word MBME, I also mean MBOME, the National Board of Osteopathic

0:43.7

Medical Examiners, right?

0:45.5

So the thing is the MBME is they love people to know the classifications of viruses.

0:50.3

So I'll say that that's something that's imperative to know.

0:52.4

Like for example, in one of the classes I held recently, I was explaining how knowing that parvo virus B-19 is a single-stranded DNA virus is a pretty high-yled thing to know for your exams. Because as you know, many DNA viruses tend to be double-stranded, right? So like for example, herpes is a double-stranded virus, right? Herpes, HSV is a double stranded virus right herpes HSV is a double

1:12.9

stranded virus it's a double stranded linear enveloped virus all right so let's jump right

1:18.4

into it right so what if they give you a question about a dentist that has herpetic weight

1:22.4

low right what is the most likely herpes virus that they have I really hope you're saying

1:26.7

divine this is probably going to be HSV 1 again can it be HSV 2 What is the most likely herpes virus that they have? I really hope you're saying divine,

1:35.0

this is probably going to be HSV-1. Again, can it be HSV-2? Absolutely, but it's probably going to be HSV-1,

1:41.3

right? So how do people get HSV-1? Well, typically, people are going to get HSV-1, you know, from saliva.

1:45.4

That's why you can see the association with dentists and dental hygienists on your exams right you can get it from respiratory secretions you can get it from saliva right

1:50.2

and it can cause many different findings right like for example on your exams it can cause

1:56.0

a cold sores on the mouth right so if you see a person that has cold soles on the mouth, right, on the lips, for example, like the outer lips, it's probably going to be HSV-1, right? But it can also cause other presentations, right? It can cause gingivostomatitis, right? So it can cause issues in your oral cavity, right? It can also, and remember, what's the drug that can cause gingerful hyperplasia? I hope you're seeing, oh, divine, that sounds a lot like cyclosporine toxicity. Remember cyclosporin is an immunosuppressant, right? It can cause eye inflammation, right? It can cause carotoconjunctivitis, right? Especially when you have these dendritic ulcers, if they use the term dendritic ulcers on your exams, think about HSV-1, right? But remember, HSV-1 is not just

2:36.1

limited to the face. It can also cause problems in the, you know, in the person's genitals, right?
