Chlorthalidone is a thiazide diuretic that uniquely raises blood calcium levels compared to loop diuretics which lower calcium levels.



There was a recent piece of literature (2020) comparing chlorthalidone versus hydrochlorothiazide. It found that chlorthalidone was no more effective than HCTZ.



Remember that chlorthalidone can raise uric acid and increase the risk of gout flares.

All right, so let's get into the drug of the day today, and that is chlorthalodone.

And this is going to be a little bit more interesting in that we've had some recent research done on chlorthalodone.

And I'll get to that a little bit, but some of the basics, brand name is thalitone.

This drug is classified as a thiozide diuretic.

So similar to hydrochlorothiozide, metolazone,

those are some of the other drugs that are going to have similar effects

to a drug like

chlorthalidone. So you can imagine its uses are going to be pretty similar to that as well.

In practice, hypertension is by far the most common use that I've seen this medication used for,

followed by probably edema swelling.

Again, we often go more towards the loop diuretics like ferrosamide for removal of fluid,

but thiaside diuretics can have that effect as well.

And then lastly, on a rare occasion, I have seen it utilized for the prevention of calcium-based renal stones because chlorthalodone actually helps retain calcium back into the body.

And so that prevents it from going out and contributing to growing stones and that type of
