So let's get straight into it.

Wilms tumor is a specific type of tumor that affects the kidneys in children, typically under

the age of five years.

Consider a Wilms tumor in a child under the age of five years presenting with a mass in the abdomen.

The parents may have noticed the mass or they may present with signs and symptoms of abdominal pain,

hematuria or blood in the urine, lethargy, fever, hypertension or weight loss.

The initial investigation of choice is an ultrasound of the abdomen to visualize the kidneys.

A CT or MRI scan can be used to stage the tumor

and a biopsy is required to identify the histology and make a definitive diagnosis.

Treatment involves surgical excision of the tumour along with the affected kidney,

which we call a nephrectomy.

Adjuvant treatment refers to treatment that's given alongside the initial management with surgery,

and this depends on the stage of the disease, the histology and whether whether it's spread. The main options are adjuvant chemotherapy and adjuvant radiotherapy.

Regarding the prognosis, early stage tumours with favourable histology have a good chance

of cure of up to 90%. Metastatic disease has a more poor prognosis.
