Today, we have an important topic to discuss with you, and at first it may not sound important, or maybe you're not quite sure what it is or what the benefit of it is, and that's the care plan.

So today we're going to be going through what a care plan is

and how it's not only important for your loved one, but how it can be beneficial to you and your

family as well. First, though, we need to explain that the type of care plan we're going to be

discussing is one that's used for home care. There are several different types of care plans,

and each one of them are

important to the care that your loved one receives and the goals that are hoped to be achieved.

But for today's discussion, we'll be focusing on care plans used for home care.

Typically, when a family makes the decision that their loved one needs extra help, or maybe

they've noticed things that have raised concerns, or maybe their doctor has recommended that the family looks

into getting them some help.

Either way, an initial assessment and care plan will be the first step in the care process

after the home care provider has been chosen.

So what is the care plan?

The care plan is part of the initial assessment or

intake for your loved one by the home care provider when they meet with you and your loved one

initially. The assessment or intake will go over several areas of the individual's abilities,

medical history, lifestyle, and their living environment. They'll usually go through the following areas.
