I think later in the program today, we'll return to the absolute shambles of the so-called homes for Ukrainians scheme.

And some astonishing comments in the House of Commons yesterday from one Conservative MP that perhaps explain why there is so little appetite to do the most

obviously decent thing. But we begin with a return to one of the most troubling and indeed

heartbreaking stories of recent years, I think it can be fairly said. With this astonishing

statement that women in England are not

safe in childbirth until the recommendations of a report on the NHS's worst maternity scandal

are implemented in full, and that is the conclusion of the author of that report, Donna Ockenden.

You'd have heard a lot over the last 24 hours about the failures at Shrewsbury and Telford Hospital NHS Trust.

But perhaps most pertinently, the conclusion that these failures were, and I quote, not unique, is what should concern us most.

It's a five-year investigation.

It concluded that 201 babies and nine mothers, forgive me, this is upsetting,

had died avoidably because of repeated failures in care over two decades. And that's that,

those are the two phrases that we focus upon first, over two decades, which includes the

birth of both of my children and and not unique. So three phrases,

repeated failures, over two decades, not unique. How do we do this? How do we do this?
