you want feedback, maybe you don't need to create an elaborate survey that people won't really feel like filling out.

You could just ask for feedback in whatever way people want to offer it.

After all, feedback is a gift,

so it may not be wise to tightly dictate the terms.

So every time I turn around, I feel like I am being asked for feedback.

The venue where I went to Disney on Ice, asked me to fill out a survey about my visit. My doctor's office

wants to know how they did. And all of this can be great. I appreciate the desire for continuous improvement.
