Why ED regional nerve blocks for hip fractures?

0:21.3

This isn't a classic EBM topic.

0:24.2

It isn't even a medical myth.

0:26.1

I mean, we love covering myths on journal jam.

0:28.6

It's not even particularly exciting.

0:31.0

So, Justin, why this topic?

0:34.0

You know, I think this is a good topic for a couple reasons. And I think the evidence is

0:38.9

messier than what we thought when we originally went at this topic. But what we really wanted,

0:44.3

0:48.8

how evidence-based medicine can have, you know, a real positive impact on our patients,

0:55.6

even if it's for what seems like a pretty small question. And then second, because the evidence isn't perfect, I mean,

1:01.2

1:05.9

in the real world. So I think it gives us a good opportunity to just talk about what we do in the face of

1:11.9

imperfect evidence, basically every day in our practice. Yeah. And in this case, you know,

1:16.9

pain management is all the rage these days with the opiate epidemic still raging. And we shouldn't

1:22.0

expect zero pain in all our emergency department patients. But oligo analgesia is still a problem in the emergency department, especially in our older patients.

1:32.3

Yeah, I understand that there's a few pretty good papers showing exactly that, that, you know, we don't do a

1:37.8

great job with pain control, especially in the elderly.
