0:00.0 | Hello and welcome to the Zero to Finals podcast. |
0:08.8 | My name is Tom and in this episode I'm going to be going through hernias. |
0:13.2 | And you can find written notes on this topic as always at zero to finals.com |
0:17.3 | slash herniers or in the general surgery section of the zero to finals surgery book. |
0:23.1 | So let's get straight into it. |
0:25.4 | Herniers occur where there is a weak point in a cavity wall, usually affecting the muscle or the |
0:31.5 | fascia. |
0:32.6 | This weakness allows a body organ, for example the bowel that would normally be contained within that cavity |
0:38.7 | to pass through the cavity wall. Let's talk first about the presentation. There are many |
0:45.1 | types of hernias that present differently depending on where they are and what organs are involved. |
0:50.7 | The typical features of an abdominal wall hernia are a soft lump protruding through the abdominal wall. |
0:57.6 | The lump may be reducible, meaning it can be pushed back into the normal place. |

1:31.9 | reduced back into the proper position, meaning it's irreducible. The bowel that's coming through the |
1:37.8 | hernia is trapped in the herniated position. Incarceration can lead to obstruction and strangulation of the hernia. |
1:46.2 | Obstruction is where the hernia causes a blockage of the passage of feces, fluids and gas |
1:52.2 | through the bowel. Obstruction presents with vomiting, generalized abdominal pain and |
1:58.0 | absolute constipation. Absolute constipation means not passing feces or phletus. |
