In 2016, toddler Dennis “Boo” Vaughan Jr. and his three siblings were removed from their parents’ custody due to concerns of substance abuse and domestic violence. They were ultimately placed in the Laconia, New Hampshire, home of their maternal grandmother, Sherry Connor. Their mother, Danielle Vaughan, had experienced her mother’s abuse firsthand and knew Sherry was mistreating and abusing her children. 

Over the next two years, DCYF received at least 27 reports of child abuse and neglect against the Vaughan children. Despite these warnings from Danielle and several others, including mandatory reporters, DCYF continually overlooked the increasingly egregious and shocking ways Sherry treated her grandchildren. Only after Sherry accidentally sent Danielle audio of one abusive incident did DCYF act, and it was nowhere near enough.
