Summary

And now for the detailed PART TWO of the definitive discussion/debate on CAC value with Dr. Malcolm Kendrick, GP - and Dr. Scott Murray, Cardiologist.

Note again that this is PART TWO which covers the detailed technical discussions on each point raised by Malcolm's blog post last month - Part One available here covers the core on whether one should get a Calcium Scan and their CAC Score (The Big CAC Debate Part One)

Yes, yes I know the Scottish and Irish accents are fantastic but may be challenging for some to grasp every word - so full transcripts are provided :-)

Between part one and part two of these podcasts, all of your CAC questions should be answered comprehensively!

So we have Malcolm Kendrick, Dr. Extraordinaire, who pretty much agrees with everything I say and vice versa, except a slight difference of opinion on calcium scanning utility.

0:19.0

And Dr. Scott Murray, cardiologistologist who has a huge amount of experience

0:23.1

in imaging and preventative and interventional cardiology and again we'll have his own

0:28.6

opinions on calcium scanning so this is the big discussion so I thought well great to

0:34.2

see again Malcolm first of all. No, thanks.

0:39.4

And Scott, I suppose.

0:40.5

Welcome, thanks.

0:46.4

So anyway, we had a quick chat in the last few days about what structure we'd use.

0:50.0

And we reckoned it'd be good to follow the structure of Malcolm's blog because that followed a logical kind of sequence looking at the evidence on various points.

0:56.3

And if we have maybe Malcolm just given his thoughts on each point of evidence or science,

1:02.1

and then Scott, you give your opinion.

1:04.8

And then at the end, I'll just give my thoughts, maybe refer to a couple of papers that lead me that direction.

1:12.2

And then when we've gone through all the points points I think it's nine or ten of them and everyone can make

1:16.7

their own decision based on what we say then we go to part two which is should you get a

1:21.6

scan or not or who should get a scan yeah yeah ok-doke so the first thing is the soft plaque question so this comes up all the time

1:32.3

and if I had a dollar for every time it's brought to my attention or I'm asked about it I could

1:37.4

retire basically so soft plaque how important is it that CAC does not directly visualize or quantify soft plaque specifically

1:47.0

it more quantifies the calcified plaque how important is that in this overall question of the

1:54.3

utility of CAC well I think it obviously to an extent it's key I suppose, because for people who may be catching for the first time, soft plaque or I think sometimes called vulnerable plaque is the plaque that is potentially most likely to pose problems.

2:14.6

So some people call it vulnerable because vulnerable plaques can do this

2:18.7

thing called rupturing and that is the trigger for a blood clot that might block your artery completely.

2:26.2

So they're the kind of dangerous ones. I know this is taking about 5,000 million bits of information
