Hey folks Scott Weingard here and this is the M-Krit podcast. I am super excited today because I get to talk to my partner in crime on M-Krit Josh Farkas.

Josh is a pulmonary and critical care doctor in Vermont. He is the

editor-in-chief of the poem-crip blog. He is the creator of the Internet Book of

Critical Care and I think he is one of the best minds in critical care and I wanted to talk to him about a topic that might not seem on its face to be tantalizing which is how to do the critical care med rec the medicine medication reconciliation.

Oh God you know this is not crashing pulmonary embolism but it is critical

and no one teaches you this.

I went through two critical care fellowships.

No one sat me down and taught me this.

You just kind of pick it up on your own.

You never know if you're really doing it right or not.

And so I wanted to talk to Josh about this. He has an

upcoming chapter in the internet book of critical care on this topic and we

figured it would be great to just put this out there. Now Josh's chapter is still in the state of peer review and that's

another reason we wanted to do this episode is the text of Josh's chapter is

going to be on the show notes for this episode.
