Clomiphene is oral and relatively inexpensive compared to other agents that are used for infertility management.



Clomiphene is a SERM so you can anticipate that some patients may report vasomotor symptoms like flushing and hot flashes.

So with that, let's get into the drug of the day today, and that is clomaphene.

Brand name of this medication is Clomid, and this drug is categorized as a serm.

So that's a selective estrogen receptor modifier.

And you may have remembered, I believe I've covered a couple of serms,

but probably the most common serms I think of in clinical practice are raloxifine and tamoxifen.

However, clomophine is definitely used for something significantly different than those two agents.

Tamoxifen being typically breast cancer, reloxifine can be used for osteoporosis.

So clomophine is actually used to help stimulate ovulation.

So we're primarily looking at female infertility and things of that nature.

So mechanistically, how does it do that?

Well, being a CERM, you can expect that it's going to mess with estrogen receptors in some capacity.

So it binds estrogen receptors, and Clomophene actually hangs onto those receptors

or stays coupled to those receptors for a longer period of time than estrogen does.

And if you remember in the body thinking about using birth control, that's using estrogen to essentially reduce the risk of pregnancy by binding to those estrogen receptors longer.

It kind of messes up that normal estrogen negative feedback, which ultimately prevents ovulation.
