Summary

Welcome back!

First up a paper to challenge the way we think about rhythm recognition in cardiac arrest to start with, looking at the rate of VF identified on echo but not on the defibrillator.

We have a huge amount of strategies to rule out acute coronary syndrome in the UK, our next paper looks at the clinical effectiveness of these, whilst also giving us some hugely important information about the incidence of ACS in those presenting to Eds.

Finally we look at a paper quantifying the effect of hypertonic saline in those patients with a TBI.

Once again we'd love to hear any thoughts or feedback either on the website or via X @TheResusRoom.

We'll be taking a short break over the summer, but will be back in September with another Papers of the Month and Roadside to Resus, until then have a fantastic summer!

So hi, and welcome back to the recess room podcast.

I'm Simon Lang, and I'm Rob Fenwick.

And this is August 2025's Papers of the Month. Oh, yes, it is August 2025's Papers of the Month.

0:22.4

Oh yes, it is.

It's Papers of the Month leading us into the traditional recess room summer break.

I would imagine Simon Lang will be bathing in Factor 50,

0:31.6

checking out his impressive collection of practical yet stylish sunhats and sipping gently on a pinoccalada,

0:39.4

not five minutes after we finish this recording. But first, we've got some papers to have a little look up. So first up,

0:44.7

we're going to be looking at the incidents and relevance of echo visualization of occult VF.

0:49.8

This is really interesting, by the way. Then I'm going to be taking us through ACS or acute coronary syndrome strategies in the ED.

0:57.5

So which ones are best for ED length of stay, particularly is the question here.

1:01.9

And finally, should we be preferentially using hypotonic saline to lower intercranial pressure after traumatic brain injuries?

1:10.2

So cardiac arrest, acute coronary syndromes and traumatic brain injuries, what a perfect way to spend half an hour. Yeah, I'm a bit worried about my ice cubes melting for my Pina clada, so we will have to crack through this on time. It feels a little bit like the end of term at school, doesn't it? It being released back into the wild, so there'll be no roadside to recess in August, but we'll be back with that and papers in month in September. And I think we deserve the break, because it's been a pretty busy last term. We've been involved in a few different things. I'm really excited that we've been involved in developing an extracation course, which will be able to bring you more details of very shortly. And also,

1:45.4

we've got our live Roadside to Recess brought to you free of charge by Zol and we'll be sending

1:52.7

out the invites to that for our October date. So really looking forward to meeting some of you in person.

1:58.6

And on that note, a huge thanks to Zol Medical Corporation

2:01.5

for collaborating with us, not only on that event, but with a podcast in their pursuit of

2:07.1

excellent patient care and making this all free and open access medical education. So, without

2:13.6

further ado, let's crack into the podcast.

2:23.1

Right, well, Rob said this was a good one and he wasn't lying.
