Summary

Alendronate is a bisphosphonate that is used in the management of osteoporosis.



In this episode, I discuss osteonecrosis risk and what are some of the risk factors that may increase the chances of this very rare adverse effect.



Binding interactions are so critical with alendronate. They can essentially make the drug useless.



Esophageal irritation and ulceration is one of the possible complications with the use of alendronate.

So today I'm going to cover a Lendronate,

which the brand name of that medication is Phosphamax.

This is from a class of medications used in the management of osteoporosis.

So being a bisphosphonate mechanistically, how this drug works is the drug

ultimately blocks the action of osteoclass. So if you go back to physiology and you remember osteoclast and osteoblast, the easiest way I remember it is osteoblast, build bone, and osteoclass break them down.

So blast build, class, break them down.

Now, by inhibiting osteoclast, we're essentially inhibiting that

breakdown of the bone, sometimes called decreasing bone resorption. And that's going to ultimately

increase bone mineral density, which makes your bones stronger,

obviously, and helps fight off some of the negative aspects of osteoporosis. So that's how the

drug works. With administration, this is probably one of the most important patient education points.
