So let us begin.

You have a 36 year old male with a past medical history of poly substance abuse, heroin, crack cocaine, and ETOH.

Currently on methadone, presents with a chief complaint of left-hand pain.

Patient states that he was in an altercation. complaint of left-hand pain.

Patient states that he was in an altercation the night prior and injured his left

wrist after a fight.

During the interview, he was very remorseful.

He stated that he had been doing well on methadone, however he slipped last night and did three bags of heroin because he was upset following the fight.

Initial vitals revealed no fever, heart rate in the 70s, BP 125 over 75,

a respiratory rate of 18 and oxygen saturation

98% on room air.

On exam, the patient was awake and alert, pleasant, conversing appropriately.

Really the only remarkable exam finding was a swollen left wrist that was tender to palpation.

The patient was hesitant to partake in the full exam of the wrist due to the pain. So you ordered imaging at the left wrist and you move on to your next patient.
