My little post on Sensible Medicine struck a nerve, and I got a lot of emails from people telling me, well, here's an accommodation you didn't hear.

You didn't hear it from me, but here's one that I heard about.

Carbon footprint, the New England Journal of Medicine, has a perspective where they ask us

that randomized controlled trials should include as an endpoint of the study, the carbon footprint.

We'll talk about that.

We'll talk about should evidence have an expiration date, a concept that Adam and I and Palmer Green

pioneered a few years ago.

And we'll talk about a little conference in Rome called Four Words.

But first, John, you're you're there in the thick of things.

You're in the HRS conference, which is the Heart Rhythm Society.

You're probably thinking about how you can be ablating at atrofibrillation or something like that.

What are you guys talking about? What's the hot topic there at HRS? Well, the hot topic,

you would think, you would think since we don't know what causes atrial fibrillation,

how to ablate atrial fibrillation, that this is a scientific session that we would be interested

in finding the solution, but that is a scientific session that we would be interested in finding the solution,

but that is not what everybody's talking about. What everybody's talking about is a new way

to destroy cardiac tissue called pulse field ablation, and instead of heating it or freezing

it, pulse field shocks it. And this is the excitement. All of the companies are big

into this thing called PFA, which shocks the heart and kills cardiac tissue. And of course,
