I'm very pleased on this journal jam number five on cardiac troponins to have a special guest

commentator with us. We have Dr. Andrew Worcester from McMaster University.

Dr. Worcester, can you just tell us a little bit about your background in emergency medicine?

Well, first of all, Anton, thanks very much for inviting me. I'm an emergency physician. I did my

training in clinical epidemiology as well, And for the last 15 years have been

the director of research for emergency medicine at Hamilton Health Sciences. Wonderful. Let's talk a

little bit about cardiac troponin. You know, there's lots and lots of patients who present to the

ED with chest pain.

Once we've ruled out in our head, all the other potentially life-threatening causes of chest pain,

like dissection, PE, esophageal rupture, and stemmy, we're often left wondering whether the patient

in front of us with a normal or at least non-aschemic looking ECG may still have ACS.

And one of the main goals with these patients is to get them sorted out quickly.

You know, we either want to get a quick rule-in answer so that we can start treatment early

or a quick rule out so we can send them home.

Now, traditionally, we've drawn tropononents at four and eight hours or something like that
