Summary

A 67 year old woman with a history of hypertension, hyperlipidemia, diabetes, and a 25 pack year smoking history is referred your clinic and is referred for evaluation of her peripheral arterial disease. She reports pain with walking that has limited her doing some daily activities. How can you optimally manage this patient? Does she need an operation? In this episode, we will cover the basics of peripheral arterial disease, discuss the specifics of optimal medical management and dive into the nuances of when (or if) you should offer these patients an operation.

alike. This is a topic because we've just gotten quite a bit of press and publicity

recently and is one of the most common vascular diseases worldwide.

So without further ado, let's get into everyone's favorite vascular disease, drum roll please, intermittent clotication.

Okay, hopefully you can sense a little bit of sarcasm there that this is definitely not everybody's favorite topic,

but has certainly been thrust in the limelight recently, is incredibly common and has some

controversy surrounding it, so I really think it's worth reviewing. Let's get right into it.

Drew, why don't you start us off with some basics about peripheral artery disease and some

internecordication facts? Sounds good, definitely. So peripheral arterial

disease or PAD is defined as a chronic, athoscrotic, occlusive disease of the lower extremities.

It's estimated that more than 200 million individuals are currently affected by PAD worldwide,

and the overall prevalence of PAD is 4.3% and it actually increases with each decade of life.

So a person in their 40s, four person in their 40s, the prevalence in 0.9%.

And when that same person gets into their 70s, that prevalence is 14.5%.

And sadly, the prevalence rate is increased by approximately 25% over the last 10 years.

And because PAD is a chronic manifestation of systemic atherosclerosis, the risk factors are

really similar to all the other risk factors we think of when we think of afroscrotic diseases.
