0:27.5 | Chronic lung disease of prematurity, or CLDP, is also known as broncopulmonary dysplasia. |
0:34.5 | It occurs in premature babies, typically those born before 28 weeks of gestation, |
0:40.1 | and these babies suffer with respiratory distress syndrome and require oxygen and therapy |
0:45.4 | or intubation and ventilation at birth. A diagnosis of chronic lung disease of |
0:51.3 | prematurity is made based on chest x-ray changes and when the infant |
0:55.8 | requires oxygen therapy after 36 weeks gestational age. So what are the features? Low oxygen |
1:04.2 | saturations, increased work of breathing, poor feeding and weight gain, crackles and wheezes on chest excultation, |
1:12.6 | and increased susceptibility to infections, particularly respiratory tract infections. |
1:18.3 | How can we prevent chronic lung disease of prematureity? |
1:22.4 | Well, there are several measures that can be taken to minimize the risk of CLDP. |
1:27.3 | These include giving corticosteroids, particularly beta-metazone, |
1:31.6 | to the mother that shows signs of premature labour at less than 36-week gestation, |
1:37.7 | and this can speed up the development of the fetal lungs before birth |
1:41.1 | and reduce the risk of chronic lung disease of prematurely. |
1:45.1 | Once the neonate is born, the risk of CLDP can be reduced by using CPAP rather than intubation |
1:52.4 | and ventilation where possible, using caffeine to stimulate the respiratory effort, and not |
1:59.0 | over-oxygenating with supplementary oxygen. |
