An 18-year-old male presents to the trauma bay following a stab wound to the left chest. He is anxious and pale but conversant. He is bilateral breast sounds.

Femeral pulses are palpable the week. GCS is 15. Heart rate is 130. Blood pressure 88 over 60. Respiratory rate 30. An auction saturation 98% on a non-rebreeder mask.

Secondary survey reveals no other injuries. How would you like to proceed? You're at a level one trauma center with full support.

Okay, yeah, I would start with a fast exam and then a chest x-ray. I'd also like to attain at least two large bore IV access, send a full set of comprehensive labs and an older blood to the bedside immediately.

Fast exam shows a large paracardal tamponon with depressed cardiac function and chest x-ray is unremarkable.

Okay, in this case, I would take this person to the operating room as soon as possible. I'd also begin a blood transfusion, either whole blood or a one-to-one balance product.
