Did you know that we are facing a continuing growing cesarean section crisis? The overuse of cesarean sections, especially in the United States, is not new, but it has certainly continued to become more prevalent globally over the past 15 years. Women's ability to give birth is not greatly dependent on geography, and yet many women in low-income countries do not even have access to this method of birth when necessary. Wide variations in intervention statistics between regions tell us that something is very wrong. Joining me is Dr. Marleen Temmerman, a force in bringing the overuse and under-access of cesarean sections to the wider medical community. She is an obstetrician/gynecologist and a world leader in women's and children's health. Notably, Dr. Temmerman was the senior author on a series of focus articles in The Lancet medical journal for October 2018. She also has a strong record of diplomacy, advocacy, and clinical expertise. She also served as the director of the Department of Reproductive Health and Research for the WHO, as an elected senator for the Belgian parliament, and is the founding director of the International Centre of Reproductive Health at Ghent University, with offices in Kenya and Mozambique. 0:37.8	are most economically underserved are receiving too little of what they need and those in the most resource
0:45.8	rich countries are being oversold. The use of Cessarian sections and the overuse
0:51.8	particularly in the US is not new.
0:55.2	This issue has been obvious since the 1980s.
0:59.1	Ironically at that time the C-section rate was considered to be too high when it was
1:04.0	escalating to 20 percent but now that would almost be a dream goal
1:08.6	compared to the over 30 percent average we have, though according to the World Health Organization,
1:15.3	the ideal is really between 10 and 15%, but no more than 19% for maximum outcome benefits.
1:23.7	But if that wasn't already enough of a crisis,
1:26.1	Cessarian section births almost doubled between 2,000
1:30.1	and 2015 globally in high income settings with countries like Brazil having an
1:35.7	over 60% sesection rate while women in low income countries often don't have
1:40.9	access to this method of birth at all even when it's needed for saving a woman's
1:45.4	life.
1:46.3	The wide variations reported between regions, within countries, and even within counties in a country, as we see widely happening in the US and other places,
1:55.7	tells us that something is very wrong, because women's ability to give birth is not greatly dependent

2:05.8	that Cessarian section use is not being evidence-based. Finally the conversation is starting to go mainstream as it needs to for change to happen, and my
